The verdict

Cuts post-exercise soreness (WMD −0.56 VAS points, 95% CI −0.84 to −0.27) and creatine kinase (−137 U/L, 95% CI −239 to −36), and adds ~4.1° of range of motion (95% CI 1.45 to 6.75).

Two independent meta-analyses (10 RCTs, 2022; 14 RCTs / 349 subjects, 2024) agree in direction and magnitude on soreness, CK and ROM. Downgraded only by small trials and heterogeneous formulations — not enough to drop below strong.

How it works

Inhibits NF-κB and COX-2 signalling, blunting the IL-6/TNF-α cascade that drives secondary damage after eccentric work; free curcumin is poorly absorbed, so piperine (CYP/UGT inhibition) or a lipid/phytosome carrier is required.

Who it helps, and who it doesn't

Untrained or returning athletes and anyone doing unaccustomed eccentric work show the largest effect; the meta-analysis found benefit concentrated in untrained subjects and in supplementation runs longer than one week.

Timing

Safety flags

  • · Drug-induced liver injury is documented: 10 DILIN cases, 1 fatal; 7 of 10 carried HLA-B*35:01 and piperine-containing products were implicated
  • · Increases oxalate excretion — avoid with a history of calcium-oxalate kidney stones
