Elderberry (Sambucus nigra)
Commercially heat-treated, membrane-filtered black elderberry fruit extract standardised to ~15% anthocyanins / ~22% polyphenols. Never home-pressed juice. Syrup or capsule both work.

First results, or a theory about how it works
Not moderate, despite headline numbers. The pooled duration CI nearly touches zero (−4.16 to −0.10), certainty was rated low to very low, the positive trials are small (n=60, n=312) and industry-linked, and the only FDA-approved, PCR-confirmed, investigator-initiated trial was negative on both primary and secondary outcomes. Direction is promising; the evidence base is not yet convincing.
Across 5 small studies colds were about 2 days shorter, but the one well-run flu study found no benefit at all.
Pooled across 5 small trials, cold duration was 2.13 days shorter (95% CI −4.16 to −0.10) — but the one PCR-confirmed, independently funded influenza trial found no benefit at all (5.3 vs 4.9 days, p=0.57).
Anthocyanins bind viral haemagglutinin and block host-cell entry in vitro, and shift cytokine signalling toward faster resolution. The proposed mechanism is antiviral plus immunomodulatory, but human pharmacokinetics for anthocyanins are poor and the in vitro dose is far above what circulates.
Frequent flyers and people whose illness-onset cluster fires while they still feel well — the window where elderberry has any chance is narrow. Nobody should expect prevention: incidence was unchanged (17 vs 12 episodes, p=0.4).

Single ingredient, commercially processed Sambucus nigra. Never use home-pressed elderberry — raw berries, leaves and seeds contain cyanogenic glycosides.

A six-month count of plain elderberry. Check the anthocyanin percentage on the label — around 15% is what the standardised extracts in trials carried.

Each diamond is a study's best estimate. The thin line shows how sure the study was: the shorter the line, the more certain the result.

The studies
- 1.Elderberry for prevention and treatment of viral respiratory illnesses: a systematic reviewBMC Complementary Medicine and Therapies · 2021 · rct · 5 experiments · n = 473
Cold duration −2.13 days (95% CI −4.16 to −0.10), severity −13.69 Jackson points (−24.54 to −2.84), influenza recovery −2.68 days (−5.23 to −0.13). Certainty low to very low. Found no evidence that elderberry overstimulates the immune system.
- 2.Elderberry Extract Outpatient Influenza Treatment for Emergency Room Patients Ages 5 and Above: a Randomized, Double-Blind, Placebo-Controlled TrialJournal of General Internal Medicine · 2020 · rct · 1 experiments · n = 87
Negative. Time to symptom alleviation 5.3±3.6 days on elderberry vs 4.9±2.8 on placebo (p=0.57); complete resolution 8.6±3.9 vs 8.7±3.8 (p=0.87). Authors found no evidence of benefit on influenza duration or severity.
- 3.Elderberry Supplementation Reduces Cold Duration and Symptoms in Air-Travellers: A Randomized, Double-Blind Placebo-Controlled Clinical TrialNutrients · 2016 · rct · 1 experiments · n = 312
Total cold episode days 57 on elderberry vs 117 on placebo (p=0.02); mean duration 4.75 vs 6.88 days; symptom score 247 vs 583 (p=0.02). Incidence was not reduced (12 vs 17 episodes, p=0.4).
- 4.Randomized Study of the Efficacy and Safety of Oral Elderberry Extract in the Treatment of Influenza A and B Virus InfectionsJournal of International Medical Research · 2004 · rct · 1 experiments · n = 60
Symptoms relieved on average 4 days earlier than placebo with 15 mL syrup four times daily for 5 days; rescue medication use significantly lower. Small and industry-linked.
- 5.Poisoning from Elderberry Juice — CaliforniaMorbidity and Mortality Weekly Report · 1984 · rct · n = 11
Eleven people fell ill and eight were hospitalised after drinking juice pressed from elderberries together with leaves and branches; nausea, vomiting, cramps, weakness and altered consciousness. Leaves, bark, buds and roots contain a glucoside that can yield hydrocyanic acid.
