L-carnitine

L-carnitine L-tartrate (LCLT) for recovery — 3 g LCLT delivers ~2 g elemental carnitine. Acetyl-L-carnitine (ALCAR) if the target is fatigue/cognition rather than muscle soreness.

Some good studies, not many repeats

Why this rating

Consistent, replicated meta-analytic effects on damage markers and soreness, plus a clean 5-week RCT — but the effect sizes are on blood markers and questionnaires, the trials are small (284 participants across 14 RCTs), and the centenarian fatigue trial reports implausible body-composition changes (+3.8 kg muscle in 6 months) that undercut its credibility.

In short

Across 14 studies it lowered a blood marker of muscle damage and reduced soreness the day after exercise. It helps recovery, not performance.

The bottom line

Pooled across 14 RCTs, supplementation lowered creatine kinase 48.7 IU/L at 24 h post-exercise (p=0.006) and reduced 24 h muscle soreness (WMD -6.39, p=0.001) — a recovery-marker effect, not a performance effect.

How it works

Carnitine shuttles long-chain fatty acids into the mitochondrion and buffers the acetyl-CoA pool; muscle loading also reduces exercise-induced hypoxic/oxidative tissue damage and purine degradation.

Who it helps, and who it doesn't

People with genuinely low tissue carnitine: older adults, strict vegans/vegetarians, dialysis patients, and anyone doing high eccentric loads. Well-fed omnivorous athletes have little headroom.

What to buy

What to look for
  • ✓L-carnitine L-tartrate (LCLT) for recovery — 3 g LCLT delivers ~2 g elemental carnitine. Acetyl-L-carnitine (ALCAR) if the target is fatigue/cognition rather than muscle soreness.
  • ✓2 g/day elemental L-carnitine (as 3 g LCLT). Trials span 1-3 g/day; nothing supports going higher.
  • ✓Third-party tested (USP, NSF, Informed Sport)
What to avoid
  • ✕Proprietary blends that hide the dose per ingredient
  • ✕Unstandardised extracts with no active-compound percentage
Safety notes
  • · Fishy body and breath odour from trimethylamine is common at ≥2 g/day and is the main reason people stop.
  • · Nausea, cramps and diarrhoea, dose-dependent.
  • · Chronic loading raises TMAO, a metabolite linked to atherosclerosis in human cohorts — avoid open-ended daily use without a reason.

What the studies measured

Every result, next to the line that means no effect

Each diamond is a study's best estimate. The thin line shows how sure the study was: the shorter the line, the more certain the result.

Dose
2 g/day elemental L-carnitine (as 3 g LCLT). Trials span 1-3 g/day; nothing supports going higher.
Timing
Daily with a carbohydrate-containing meal (insulin drives muscle uptake). Loading takes weeks — this is not an acute pre-workout.

The studies

  1. 1.
Effects of L-carnitine supplementation on markers of exercise-induced muscle damage in healthy adults: A systematic review and meta-analysis of randomized controlled trials
    Advanced Exercise and Health Science · 2025 · meta-analysis · 14 experiments · n = 284

    1-3 g/day reduced CK at 24 h (WMD -48.72 IU/L, p=0.006), myoglobin at 30-60 min (-41.09 ng/mL, p=0.001) and soreness at 24 h (WMD -6.39, p=0.001); LDH immediately post-exercise was not significant.

  2. 2.
L-Carnitine Tartrate Supplementation for 5 Weeks Improves Exercise Recovery in Men and Women: A Randomized, Double-Blind, Placebo-Controlled Trial
    Nutrients · 2021 · rct · 1 experiments · n = 80

2 g/day elemental carnitine for 5 weeks improved perceived recovery and soreness (p=0.021) and lowered serum CK by 28% vs placebo (p=0.016), independent of age and sex.

  3. 3.
L-Carnitine treatment reduces severity of physical and mental fatigue and increases cognitive functions in centenarians: a randomized and controlled clinical trial
    The American Journal of Clinical Nutrition · 2007 · rct · 1 experiments · n = 66

2 g/day for 6 months in centenarians cut physical fatigue (−4.10 vs −1.10, p<0.01) and mental fatigue (−2.70 vs +0.30, p<0.001); the reported +3.8 kg muscle mass gain is implausible and should be discounted.

  4. 4.
Intestinal microbiota metabolism of L-carnitine, a nutrient in red meat, promotes atherosclerosis
    Nature Medicine · 2013 · cohort · 1 experiments · n = 2,595

Gut microbes convert L-carnitine to TMAO; plasma carnitine predicted cardiovascular events only in people with concurrently high TMAO — the basis for not recommending open-ended daily loading.
